Multi-parameter continuous non-invasive monitoring of vital signs (CoNiM) has shown decrease in risk of mortality in non-intensive care unit patients compared to regular intermittent monitoring in previous studies. However, it shows controversial results on predictability on 30-days readmission rate. Most studies pointed out that the complexity of prognostic factors and risk factors may hamper the prediction for readmission. Therefore, a new form of transitional care service from hospital is suggested to remotely monitoring patients’ condition and to detect if a patient needs an early intervention.
